What We Do
Back Nine Nurses will work closely with future residents, their families, and assisted living community team members to provide highly personalized, coordinated, and appropriate referrals to Assisted Living Communities in the state of Massachusetts.
Why Team with Back Nine Nurses
We are family advocates and do not request or receive commissions when referring families. We consider all Communities that most closely align with a family’s needs and preferences at any point in their journey. We work closely with Families, Elder Law Offices, Financial Planners, Assisted Living Communities and Healthcare Organizations to provide solutions. As experienced Assisted Living Registered Nurses, we are acutely aware of how important it is for new residents, their families, and Community team members to experience the “right fit” and enjoy as seamless a move-in process as possible.
Back Nine Nurses offers the following services when providing professional assisted living community referrals:
A completed Comprehensive Assisted Living Resident Assessment utilizing each community’s specific assessment tool
Assistance with the coordination and creation of the following documents:
• Durable Power of Attorney Form
• Healthcare Proxy Form
• MOLST
• Insurance Cards
• Physician Clearance Form
• Medication List
• Diet Form
• Personalized Resident Biography
The prospective residents we refer to your community are offered the following services:
A free initial 20-30 minute consultation to learn about their unique situation
A personalized assisted living comprehensive nursing assessment
Identification of assisted living communities that most closely align with their identified needs based on the assessment, goals, desired geographic location and budget
Arranged tours, accompanied by Back Nine Nurses, for up to three of the most appropriate communities; post-visit review provided
Customized questions to address during tours based on the individualized assessment as well as our nursing knowledge regarding assisted living day-to-day operations and Massachusetts regulations
Assistance with compiling the necessary paperwork prior to moving into a community
Review of the Service Plan generated by the Community to ensure the incorporation of the initial assessment findings, goals, needs and preferences
Follow-up with residents and families within thirty days of commencement of residency
to ensure that current services and preferences are being fulfilled; we will advocate accordingly
Self-advocacy education provided for residents and families in order to promote continued health and wellness in their new Community
